Wie man „gescheiterte Behandlungen“ liest
Dosis + Dauer + Dokumentation. Stellen Sie — idealerweise mit Hausarzt oder Psychiaterin — eine schriftliche Historie jedes Antidepressivums und jeder Augmentation der aktuellen Episode zusammen: Substanz, Dosis, Dauer, Ergebnis, Nebenwirkungen, dazu jede Psychotherapie. „Adäquater Versuch“ heißt ausreichende Dosis über ausreichende Zeit; eine Woche auf Anfangsdosis zählt in der Regel nicht. In Vorab-Genehmigungssystemen (Schweiz, Belgien, Finnland, Tschechien jenseits der Kriterien) ist dieses Dokument buchstäblich das, was der Kassenarzt liest — Ihre schriftliche Medikationshistorie ist das wertvollste Dokument, das Sie besitzen.
Zugelassen heißt nicht finanziert: das Norwegen/England-Paradox
Zulassung ist europäisch, Bezahlung national. Norwegen lehnte Spravato dreimal ab (2021–2022) — und tat dann, was kein anderes Land getan hat: Seit dem 25. August 2025 finanziert es generisches IV-Ketamin für dieselbe Patientengruppe öffentlich, mit Registernachverfolgung und Re-Evaluation bis Ende 2028. England erlebt dasselbe Paradox im Kleinen: NICE sagte nein, Schottlands SMC ja — dieselbe zugelassene Medizin ist nördlich der Grenze finanziert und südlich davon Privatsache. Merksatz: Fragen Sie nie, ob ein Medikament „zugelassen“ ist; fragen Sie, wer die Behandlung bezahlt.
Das praktische Playbook
- In Label-Systemen (Deutschland, Schottland) verschreibt die Psychiaterin innerhalb der Kriterien; die Finanzierung steckt in der Abrechnung.
- In Vorab-Genehmigungssystemen (Schweiz, Belgien, Finnland, Tschechien jenseits der Kriterien) reicht das Zentrum vor der ersten Dosis einen schriftlichen Antrag bei Ihrer Kasse ein — das kann Wochen dauern, also früh anstoßen lassen.
- In krankenhausfinanzierten Systemen (Frankreich, Spanien, Italien, Portugal) ist das Krankenhausprotokoll bzw. die Apothekenkommission der eigentliche Türsteher.

In den meisten kontinentaleuropäischen Ländern ja — Deutschland, Niederlande, Polen, Schweiz, Belgien, Italien, Irland, Spanien, Finnland, Tschechien, Portugal und Dänemark finanzieren es öffentlich, fast immer unter strengen Therapieresistenz-Kriterien. England ist die große Ausnahme (NICE lehnte ab; Schottland akzeptierte), Norwegen lehnte Spravato ab und finanziert stattdessen generisches IV-Ketamin.
Was zählt als „gescheiterte Behandlung“?
Dosis + Dauer + Dokumentation: ein adäquater Versuch heißt ausreichende Dosis über ausreichende Zeit, schriftlich festgehalten mit Substanz, Dosis, Dauer, Ergebnis und Nebenwirkungen. Mein Land erstattet — warum ist der Zugang trotzdem schwer?
Erstattung ist eine Finanzierungsentscheidung, keine Kapazitätsentscheidung. Der Engpass sind die wenigen Zentren (die Überwachungspflicht kostet Raum und Personal) und Psychiater, die den Pfad kennen.
Was, wenn ich außerhalb der Kriterien liege?
