What must the certificate from my doctor confirm?

Per the hospital's published criteria: ICD-10 diagnosis F33.1 or F33.2; treatment resistance (no response to at least two adequate antidepressant trials in the current moderate-to-severe episode, and no more than five antidepressants tried in this episode); at least a second depressive episode; current episode lasting at least six months; exclusion of borderline personality disorder and bipolar disorder; no history of addiction; and contraindication, resistance or non-consent to ECT.

Is Spravato used alone?

No. Per the hospital's information and the drug's SmPC, within the program esketamine is used together with a second SSRI/SNRI antidepressant; otherwise reimbursement is not possible.
