The Zentralinstitut fuer Seelische Gesundheit (Central Institute of Mental Health, CIMH) in Mannheim is one of Germany's leading psychiatric research and care institutions. Since 11 July 2025 it runs - together with the OVID day clinic in Berlin and under the direction of Prof. Dr. Gerhard Gruender (Dept. of Molecular Neuroimaging) - the first Compassionate Use / Expanded Access Programme for psilocybin in the European Union, for adults with treatment-resistant depression in justified exceptional cases. Treatment is inpatient-only: single doses of psilocybin (usually 25 mg, PEX010 by Filament Health) embedded in psychotherapeutic preparation and follow-up. CIMH led the EPIsoDE study (2021-2024, with Charite and the MIND Foundation), Germany's largest psilocybin TRD trial, and currently recruits for LSD (MM120) and deuterated-psilocin (CYB003) trials.

Common questions

Who can be treated with psilocybin at ZI?

Individual adult patients with treatment-resistant depression, in justified exceptional cases only: serious illness, no satisfactory treatment with approved drugs, and no possibility of joining a clinical trial; each case gets a benefit-risk assessment.

How do I apply?

How long is the wait?

The institute states it receives very many inquiries and that considerable waiting times can occur; demand is expected to significantly exceed capacity.

Is the treatment outpatient?

No - psilocybin treatment within the CU programme takes place exclusively on an inpatient basis, with additional outpatient appointments for eligibility, preparation and follow-up.

Can I get psilocybin outside the programme or trials?

No - classic psychedelics in Germany are only permitted within officially approved clinical trials or the Expanded Access Programme; they remain controlled under the BtMG otherwise.
